Purpose. This template is used by supervisors to plan and record the crew toolbox talk held at the start of each shift or before a significant task.

1.When to use this template

A toolbox talk must be held at the start of every shift and before every planned shutdown, clothing change or non-routine task. The supervisor leads the talk, and every crew member present signs the record. Completed records are filed with the shift log and reviewed by the HSE team monthly.

2.Content

The talk covers the plant status and handover items, the tasks planned for the shift, the hazards and controls for those tasks, any live permits and isolations, recent incidents and lessons, and environmental watch points such as effluent or stack readings near limits. Ask the crew for hazards they have noticed and record them.

3.Safety topic

Each talk includes one safety topic from the monthly schedule, such as isolation, nip points, mobile plant, manual handling or heat stress. The supervisor will explain the topic in plain language with an example from the site. Record the topic and any questions.

4.Actions

Record any action arising, such as a hazard to be fixed or a work order to be raised, with the person responsible and the date. Actions that cannot be closed by the crew must be entered in the Incident and Hazard Register.

5.Attendance

Every person present must sign. Anyone who joins the shift later must be briefed individually and sign the record. Contractors working in the area must attend or be briefed.
